29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equal or exceed a dose of fifty percent of the permissible daily noise exposure limit the employer did not develop and implement a monitoring program:    (a)  Welding Areas:  On or about February 22, 2018 the employer had not conducted noise monitoring at this location to determine which employees should be in a hearing conservation program.  An employee (Welder) was exposed to continuous noise at a dose of 78 percent of the permissible daily noise exposure limit for a twelve hour shift or an average of 88.2 dbA.  Calculations were based on a 12-hour shift with a reduced fifty percent Action Level of 84.1 dbA during the 578 minute sampling period on April 5, 2018.  Calculations included a zero exposure for the 142 minutes not sampled and a 2 dB error factor.    (b)  Welding Areas:  On or about February 22, 2018 the employer had not conducted noise monitoring at this location to determine which employees should be in a hearing conservation program. An employee (Welder) was exposed to continuous noise at a dose of 78 percent of the permissible daily noise exposure limit for a twelve hour shift or an average of 88.2 dbA.  Calculations were based on a 12-hour shift with a reduced fifty percent Action Level of 84.1 dbA during the 578 minute sampling period on April 5, 2018.
