29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking-working surface: a) At the facility, on or about October 18, 2017 and at times prior to, the loading dock did not have a mid-rail as part of the guardrail system, exposing employees to a fall hazard of 50.5 inches onto concrete floor.
