29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. On or about October 24, 2017, and at times prior thereto, employees were performing servicing and maintenance activities, such as, but not limited to, cleaning refractory material from under, around, and near DISA casting line equipment which was not locked out to control hazardous energy. The employer did not ensure that the employees engaged in the covered activities had received authorized lockout/tagout training.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):   a. On or about October 24, 2017, and at times prior thereto, employees were exposed to pinch points and ingoing nip point hazards while performing cleaning activities in order to prevent buildup of refractory material underneath the PMC to SBC transfer conveyor equipment at the DISA casting line. The employer did not implement energy control steps as the equipment was not shut down or turned off prior to performing the servicing work [per the 1910.147(d)(2)-(6)requirements].     b. On or about October 24, 2017, and at times prior thereto, employees were exposed to moving parts, pinch points, and ingoing nip point hazards while performing cleaning activities in order to prevent buildup of refractory material underneath the feed belt conveyor equipment on the second floor at the DISA casting line. The employer did not implement energy control steps as the conveyor was not shut down or turned off prior to performing the servicing work [per the 1910.147(d)(2)-(6)requirements].

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.     a. On or about October 18, 2017, and at times prior thereto, employees were reaching into the unguarded section of the second floor DISA feed belt conveyor in order to retrieve a sample(s) of sand for testing. The unguarded area exposed employees to a caught in hazard.         c. On or about October 9, 2017, and at times prior thereto, employees operated a drill press in the DISA maintenance area to drill, tap, and clean out holes in material to support the DISA production equipment. The unguarded chuck and cutting bit on the drill press exposed employees to a caught in hazard.       d. On or about October 9, 2017, an employee operated a drill press to drill and tap parts at the Number 1 CNC in the Upper End department. The unguarded chuck on the drill press exposed employees to a caught in hazard.

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  a. On or about October 24, 2017, the employer did not ensure that the working space in front of multiple electrical panels and disconnects located in the foundry remained clear from the storage of material and equipment.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons.   a. On or about October 18, 2017 employees had an occupational exposure of blood while cleaning and sanitizing the floor, stairs, and machine after an employees hand and arm was amputated by a rotating aerator shaft. The employer did not ensure that each employee involved in the cleanup of the blood had been offered a Hebatitis B vaccine.
