29 CFR  OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to molten metal explosions during slagging operations at the DISA furnace when using tools that were not free of moisture.   a.  On or about October 5, 2020, at Casting Solutions 2345 Licking Rd, Zanesville, OH 43701, the employer did not ensure that the refractory coating applied to the slag spoon was free of moisture before allowing the employee to use the spoon to remove slag from the Disa Furnace; thereby exposing employees performing activities such as removing slag from the receiving spout of the Disa Furnace to thermal burns from molten metal and moisture explosions.
