29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally: The employer is failing to protect employees from fall hazards associated with improper use of extension ladders as means of access/egress while performing residential construction activity. The most recent occurrence of this was observed on Oct 25, 2017 at the new home being built at 901 Edgewater Dr. Papillion, NE where the employer has a crew of 5 employees using an aluminum ladder set up on the rake on the west side of the roof as means of access/egress to the top roof of the home; the rake does not present the top rails of the ladder an even surface to rest on and create a tipping hazard.

The employer is failing to protect employees from fall hazards associated with performing residential construction activity while exposed to falls 6 feet or greater. The most recent occurrence of this was observed on Oct 25, 2017 at the existing residential home located at 901 Edgewater Dr. Papillion, NE where the employer has a crew of 5 employees exposed to fall hazards while re-roofing an existing residential home. Urbina Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 907035, citation number 1, item number 1 and was affirmed as a final order on 7/25/2013, with respect to a workplace located at 13953 Arbor Cir, Omaha, NE 68122.
