29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Residential garage - 944 Matchpoint Dr., Ammon, ID 83406: On or about August 7, 2018 and at times prior thereto; three employees were preparing the roof of a detached garage for with a 7:12 pitch tearing off damaged roofing material while working at heights from 11-22 feet, and they were not protected by a personal fall arrest system or other means of fall protection. Note* Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Residential garage - 944 Matchpoint Dr., Ammon, ID 83406: On or about August 7, 2018 and at times prior thereto; roofers accessed the roof of a detached garage, 11 feet at the eave using extension ladders that were less than three feet above the roofline.
