29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system. The employer is failing to protect employees from fall hazards. This was most recently documented on or about July 28, 2022 at address located at 7506 Grover St. Omaha, Nebraska where the employer has employees exposed to fall hazards while re-roofing an existing residential home. Urbina Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1273005, citation number 2 , item number 1 and was affirmed as a final order on 04/30/2018, with respect to a workplace located at 901 Edgewater Dr. Papillion, Nebraska.

29 CFR 1926.1053(b)(10):The top of a non-self-supporting ladder shall be placed with the two rails supported equally unless it is equipped with a single support attachment. The employer is failing to protect employees from fall hazards. This was most recent documented on or about July 28, 2022 at 7506 Grover St. Omaha Nebraska with improperly use of an extension ladder as means of access/egress while performing residential construction reroofing activity using an aluminum ladder set up on the rake along the side of the roof as means of access/egress to the top roof of the home creating unequal support on roof more then 10FT in height. Urbina Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(10), which was contained in OSHA inspection number 1273005, citation number 1 , item number 1 and was affirmed as a final order on 04/30/2018, with respect to a workplace located at 901 Edgewater Dr. Papillion, Nebraska.
