29 CFR 1926.1053(b)(4): The employer did not ensure ladders were used only for the purpose for which they were designed. (a)Integrity Construction, Inc. at 601 South Main Avenue, Marion, SD 57043: On and prior to October 27, 2017, the employer did not ensure that each employee was protected from falls from heights in that employees using the Werner MT-22 telescoping multi-use ladder in a stepladder conformation to access the roof of a residential home did not follow the manufacturer's instruction for use when crossing over from the ladder to the roof. This condition exposed employees to fall hazards of approximately 12 feet.

29 CFR 1926.501(b)(13): The employer did not ensure each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system. (a)Integrity Construction, Inc. at 601 South Main Avenue, Marion, SD 57043: On and prior to October 27, 2017, the employer did not ensure that each employee was protected from falls from heights in that two employee were installing roof sheathing without utilizing an appropriate fall protection system. This condition exposed employees to fall heights of approximately 13-16 feet. Integrity Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 987983, citation number one, item number two and was affirmed as a final order on August 22, 2014 with respect to a workplace located at 5517 Oak Crest Place, Sioux Falls, SD 57107.
