29 CFR 1926.453(b)(2)(v): A safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a)Integrity Construction, Inc. at 3788 North Yukon Avenue, Sioux Falls, SD 57107: On or about and at times prior to September 3, 2024, the exposing employer did not ensure that an employee was protected from falls while conducting framing activities while working from a Skypower aerial lift platform, model number 600S, serial number 0300128511, located on the north side wall of a multi-family residential building and the employee was not using a personal fall protective system. This condition exposed the employee to fall and ejection hazards greater than ten feet. (b)Integrity Construction, Inc. at 3788 North Yukon Avenue, Sioux Falls, SD 57107: On or about and at times prior to September 3, 2024, the exposing employer did not ensure that an employee was protected from falls while conducting framing activities while working from a Skypower aerial lift platform, model number 600S, serial number 0300128511, located on the northeast side wall of a multi-family residential building and the employee was not using a personal fall protective system. This condition exposed the employee to fall and ejection hazards greater than ten feet. Integrity Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v) which was contained in OSHA inspection number 1741363, citation number 1, item number 4a, and was affirmed as a final order on August 9, 2024, with respect to a workplace located at 3780 Yukon Avenue, Sioux Falls, SD 57107.

29 CFR 1926.453(b)(2)(iv): Employees working in an aerial lift were sitting or climbing on the edge of the basket: (a)Integrity Construction, Inc. at 3788 North Yukon Avenue, Sioux Falls, SD 57107: On or about and at times prior to September 3, 2024, the exposing employer did not ensure that employees were protected from falls while conducting framing activities while climbing on the railing of an aerial lift platform located on the north wall of a multi-family residential building. The employee was climbing on an aerial lift platform railing which was raised more than 25 feet high to gain access to the aerial lift from a framed wall top plate and an employee was not using a personal fall protective system. This condition exposed the employee to fall hazards greater than ten feet. Integrity Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv) which was contained in OSHA inspection number 1741363, citation number 1, item number 4b, and was affirmed as a final order on August 9, 2024, with respect to a workplace located at 3780 Yukon Avenue, Sioux Falls, SD 57107.

This condition exposed the employees to fall hazards greater than six feet. Integrity Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1741363, citation number 1, item number 5b, and was affirmed as a final order on August 9, 2024, with respect to a workplace located at 3780 Yukon Avenue, Sioux Falls, SD 57107.
