Employees performing sheathing work from a 5:12 pitch roof are not being protected from 11 feet and higher fall hazards by a fall protection system.   The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on October 26, 2017, when employees are sheathing a roof of a two-story residential building at Lot 67/68 Lucena Lane, Jacksonville, Florida without fall protections and are exposed to fall hazard of 11 feet above the surrounding ground.  Desouza Framing Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 943936, citation number 2, item number 2 and was affirmed as final order on December 9, 2013, with respect to a workplace located at 437 Forest Meadow Lanelot 693, Orange Park, Florida.    Desouza Framing Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 955071, citation number 1, item number 2 and was affirmed as final order on April 29, 2014, with respect to a workplace located at 4310 Eagle Landing Place, Middleburg, Florida.
