29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets: On or about October 26, 2017, at the above address employees were performing roofing work on a commercial building. The employee on the ground who was loading debris into the dumpster was not wearing a hard hat and was exposed to being struck by debris that was being thrown off the roof by other employees. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. On or about October 26, 2017 at the above address employees were performing roofing work on a commercial building. Employees were not utilizing fall protection and were exposed to a fall hazard of approximately 12.2 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Ultimate Roofing Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.501(b)(13), which was contained in OSHA inspection number 1237889, citation number 1, item number 2 and was affirmed as a final order on July 19, 2017, with respect to a workplace located at 29-41 White Birch Lane, Westmont, IL 60559.
