Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    Monarch Landing, 2047 Monarch Drive, Naperville, Illinois 60563 - on or about December 4, 2018, fall protection systems were not provided for employees walking at the open-sided edge of the sloped roof where they stepped onto/off the ladder.  Employees were exposed to fall hazards up to 10 feet to the flat roof below.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:    - as part of a complete personal fall arrest system which maintains a safety factor of at least two [per 1926.502(d)(15)(i)]; and,  - under the supervision of a qualified person [per 1926.502(d)(15)(ii)]:     Monarch Landing, 2047 Monarch Drive, Naperville, Illinois 60563 - on or about December 4, 2018, the flat-mounted temporary anchors used as part of personal fall arrest systems were not installed in accordance with the manufacturer's specifications, in that the minimum number of fasteners were not in place. The anchorage was not capable of supporting at least 5,000 pounds in the event of employee's fall.  Employees were exposed to fall hazards of up to 50 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
