29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about June 1, 2020, at the above addressed jobsite, employees performing residential roofing operations, approximately thirty-five (35') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Ultimate Roofing Solutions Inc. was previously cited for an equivalent standard of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1364295, citation number 1, item number 1, and was affirmed as a final order on January 29, 2019, with respect to a workplace located at 2047 Monarch Dr. in Naperville, IL.  Ultimate Roofing Solutions Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1342315, citation number 1, item number 1, and was affirmed as a final order on November 15, 2018, with respect to a workplace located at 13969 Stonehenge Dr. in Orland Park, IL.  Ultimate Roofing Solutions Inc. was previously cited for an equivalent standard of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1273349, citation number 2, item number 1, and was affirmed as a final order on December 6, 2017, with respect to a workplace located at 987 S. Randall Rd. in Geneva, IL.  Ultimate Roofing Solutions Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1237889, citation number 1, item number 2, and was affirmed as a final order on July 19, 2017, with respect to a workplace located at 29-41 White Birch Lane in Westmont, IL.

Ultimate Roofing Solutions Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1273349, citation number 1, item number 1, and was affirmed as a final order on December 6, 2017, with respect to a workplace located at 987 S. Randall Road in Geneva, IL.
