29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 2710 China Lake Drive, Saint Louis, MO 63129, on or about 10/24/2017, workers were observed on a Werner Aluminum extension ladders to access to an upper landing surface and ladder's side rails did not extend at 3 feet above the upper landing surface being accessed.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:  JOB SITE: 2710 China Lake Drive, Saint Louis, MO 63129, on or about 10/24/2017, workers were observed climbing up a Werner Aluminum extension ladder, and were not grasping the ladder with at least one hand when progressing up and/or down the ladder.  The worker was carrying and holding material with one hand during the climb up the ladder.  The workers were exposed to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 2710 China Lake Drive, Saint Louis, MO 63129, on or about 10/24/2017, workers were observed on the top of a 6:12 pitch roofing, activity installing new roofing materials, approximately 9' 5" and 20' 9" eave to ground level and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.  J & M SUPERIOR ROOFING, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1230733, citation number 1, item number 2, issued on 5/19/2017, and was affirmed as a final order on 10/13/2017, with respect to a workplace located at 2162 Olde Winery Road, Saint Louis, MO 63129.
