At a jobsite located at 6732 Holmes, Kansas City, Missouri, an employee was exposed to struck by hazards.  An employee was working in a trench. The employee was not protected from excavated materials that posed a hazard of rolling into the excavation. The spoil pile was within a foot from the edge of the trench.      Havens Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.651(j)(2) which was contained in OSHA inspection number 936669, citation number 1, item 2, and was affirmed as a final order on October 18, 2013, with respect to a workplace located at or near the intersection of N Hull Ave and NW Old Tiffany Springs Rd, at a jobsite the employer called Reserve at Tiffany Woods, in Kansas City, Missouri.
