29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): Site: In the facility, 5 Otis Street, Westborough, MA 01581 on 2/14/2018 and at times earlier the Energy Control procedure for the Small Erema did not contain: (a) A specific statement of the intended use of the procedure; (b) Specific procedural steps for each method of electrical isolation (i.e. local and at the main, control circuit (110volt) and main supply (480 volt)) and when it is required; (c) Specific procedural steps for dissipating thermal energy (temperatures from 0-608 degrees Fahrenheit) in the reclaimer and when it is required; (d) Specific procedural steps for isolation of pneumatic energy (0-58 psi) and when it is required; (e) Specific steps to secure the mechanical energy (rotation) of the blade assembly (i.e. blocking) in the reclaimer; (f) Specific steps outlining the correct sequencing of hazardous energy isolation (what is deenergized, first/last, what is reenergized first/last); (g) Specific steps to identify and remove/replace all guards in the proper sequence; (h) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them (which EEs, e.g. mechanics, electricians, production technicians, machine operators, etc. any or all) and; (i) Specific requirements for testing the Small Erema to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. Inteplast Group Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(4)(ii)), which was contained in OSHA inspection number 976716, citation number 1, item number 1 and was affirmed as a final order on 6/20/2014, with respect to a workplace located at 455 Somerset Avenue, North Dighton, MA 02764.

(b) Site: In the facility production area, Line 11, 5 Otis Street, Westborough, MA 01581 on 2/12/2018 and at times earlier machine operators serviced the Automatic Scrap Regrinder (ASR) by performing tasks such as, but not limited to, setting up for color changes without having received required training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. Inteplast Group Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(7)(I)(A), which was contained in OSHA inspection number 976716, citation number 1, item number 2 and was affirmed as a final order on 6/20/2014, with respect to a workplace located at 455 Somerset Avenue, North Dighton, MA 02764.
