29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about August 14, 2024, and at times prior thereto, in the Profile Plant jobsite, employees performed maintenance and repair operations without applying isolating devices to isolate all hazardous energies for the SD 11 Print Line (Printer Unit #1) while exposed to rotating parts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 7, 2024, and at time prior thereto, in the Profile Plant jobsite, employees utilized an Anilox cylinder/roller system of the Converting SD 11 Print Line (Printer Unit #2) that did not have a safety guard installed. Inteplast Group Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1656783, citation number 2, item number 1 and was affirmed as a final order on October 24, 2023, with respect to a workplace located at 101 Inteplast Blvd. IBS Plant, Lolita, TX 77971. Inteplast Group LTD was previously cited for a violation of the occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1454398, citation number 1, item 2 and was affirmed as final order on May 8, 2020, with respect to a workplace located at 455 Somerset Avenue, N Dighton, MA 02764.
