29 CFR  1910.147(c)(3)(ii):  In demonstrating that a level of safety is achieved in the tagout program which is equivalent to the level of safety obtained by using a lockout program, the employer did not demonstrate full compliance with all tagout-related provisions of this standard together with such additional elements as are necessary to provide the equivalent safety available from the use of a lockout device.  Additional means to be considered as part of the demonstration of full employee protection shall include the implementation of additional safety measures such as the removal of an isolating circuit element, blocking of a controlling switch, opening of an extra disconnect device, or the removal of a valve handle to reduce the likelihood of inadvertent energization:   On or about March 7, 2023, and times prior there to, IBS Tee-shirt Production Bag Line, TB-93, the employer failed to ensure an employee conducting press plate cleaning duties, stopped energy sources & locked out before performing the work where release of energy could cause harm.

29 CFR  1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control   On or about March 7, 2023, and times prior there to, IBS Tee-shirt Production Bag Line, TB-93, the employer failed to ensure energy control procedures were specifically clear in outlining the requirements for testing a press machine for its effectiveness of lockout and tagout devices and other energy controls, prior to allowing employees to conduct servicing of press machines.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about March 7, 2023,and times prior thereto, IBS Tee-shirt Production Bag Line, TB-93, an employee sustained disabling injuries associated with multiple finger amputations while conducting press plate cleaning duties on an inadequate-guarded, press machine.  The Inteplast Group Corporation dba Inteplast Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1454398, citation number 01, item number 02 and was affirmed as a final order on May 8, 2020, with respect to a workplace located at 455 Somerset Avenue, N. Dighton, Massachusetts.
