29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about November 8, 2017:    a) Employees installing gravel stop membrane on a commercial structure were exposed to a fall hazard of approximately 12 feet, when the employer failed to ensure that each employee was protected from the fall hazard by properly using the fall protection measure in place (personal fall arrest).     Peach State Roofing, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1066192, citation number 1, item number 1 and was affirmed as a final order on July 25, 2016 with respect to a workplace located at 2250 AL Highway 150, Hoover, AL.    Peach State Roofing, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 952057, citation number 1, item number 1 and was affirmed as a final order on March 5, 2015 with respect to a workplace located at 2026 Cumming Hwy, Canton, GA.
