29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Maintenance Shop: A 6-inch bench grinder (equipped with two grinding wheels) stationed on a work bench had a work rest on the right side that was not adjusted closely to the wheel with a maximum opening of one eighth inch. The opening was measured to be one quarter inch.  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Secretary's Office:  A relocatable power tap/surge protector listed for use with low powered loads such as computers, printers, T.V.s, radios, etc. was instead used to provide power to a microwave oven and refrigerator requiring high powered loads. The power tap, also listed to be "re-locatable" was found to be secured by screws to a block wall.   (b) Teacher's Lounge: A re-locatable power tap/surge protector listed for use with low powered loads such as computers, printers, T.V.s, radios, etc. was instead used to provide power to a vending machine.  (c) Principals Office: A re-locatable power tap/surge protector listed for use with low powered loads such as computers, printers, T.V.s, radios, etc. was instead used to provide power to a microwave oven. 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) A quart size plastic spray bottle (secondary container) of caustic "Lime-a-Way" found in a storage room in the kitchen area of the school cafeteria was not labeled, tagged, or marked with an appropriate hazard warning label. The bottle had originally contained a "hard surface sanitizer" as evidenced by the label that was on it.  (b) A quart size plastic spray bottle (secondary container) of caustic heavy duty degreaser found in a storage room in the kitchen area of the school cafeteria was not labeled, tagged, or marked with a hazard warning label. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Food service workers in the school cafeteria who were exposed to such hazardous chemicals as surface cleaners, sanitizers, degreasers, and lime scale removers had not been provided with information and training (stipulated in 1910.1200(h)(2) and (h)(3)) on hazardous chemicals in their work area.  (b) As of November 15, 2017, the employer had not trained food service workers in the cafeteria on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs), with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).   ABATEMENT CERTIFICATION IS REQUIRED
