29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:   (a) A 3-foot deep sump pump reservoir located in the "Dungeon" (Custodial Staff Office) in the basement of the school was not provided with a cover or guardrail system to prevent employees from tripping or stepping into or through the exposed opening of the reservoir.  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  a) Dungeon School Supply Storage in Basement: Cardboard boxes and plastic containers of school supplies were found improperly stacked in tiers on top of wooden storage cabinets. The boxes/containers were not stacked, blocked, interlocked or limited in height so that they were stable and secure from sliding or falling onto employees. A number of boxes were found to be listing over the edge of the cabinets.   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) A quart size plastic spray bottle (secondary container) of ammonia and water located on a counter in the cafeteria kitchen was not labeled, tagged, or marked with an appropriate hazard warning label.   (b) A quart size plastic spray bottle (secondary container) of Clorox and water located on a counter in the cafeteria kitchen was not labeled, tagged, or marked with an appropriate hazard warning label. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Food service workers in the school cafeteria who were exposed to such hazardous chemicals as surface cleaners, sanitizers, and degreasers had not been provided with information and training (stipulated in 1910.1200(h)(2) and (h)(3)) on hazardous chemicals in their work area.  (b) As of November 17, 2017, the employer had not trained food service workers in the cafeteria on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs), with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).   ABATEMENT CERTIFICATION IS REQUIRED
