29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:   On, or about, November 13, 2017, in the following locations, covers had not been re-installed on electrical enclosures:  A.    MCC Room near Plant Entrance:  Covers were not installed at the top of the Allen-Bradley 480 volt motor control center and also on horizontal wall mounted raceway located above the motor control center enclosure.  B.     Main Electrical Room:  A large Westinghouse 480 volt circuit breaker enclosure was missing to vent louvers on the right side.  C.     Small Electrical Room:  A cover was missing from the top of the Allen-Bradley 480 volt motor control center and a section of raceway was not provided with a cover above the entrance door.
