29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:      a) Job site, 301 - 307 Lincoln Avenue, Pittsburgh, PA: On or about November 29, 2017, during roofing activities, the employer did not ensure that employees used appropriate eye/face protection when operating nail guns in that at least one employee wore no type of eye/face protection and at least one other employee wore sunglasses not safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:      a) Job site, 301 - 307 Lincoln Avenue, Pittsburgh, PA: On or about November 29, 2017, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices where fall hazards of up to approximately 20 feet existed.      JYG Lopez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 966319, citation number 1, item number 1, and was affirmed as a final order on May 19, 2014, with respect to a workplace located at 8818 Royal Manor Drive, Allison Park, PA.

29 CFR 1926.20(b)(2): The employer did not institute a program of frequent and regular inspections of the job sites, materials, and equipment by competent person:   a) Job site, 301 - 307 Lincoln Avenue, Pittsburgh, PA: On or about November 29, 2017, during roofing activities, the employer did not ensure that a system was in place to identify and correct hazards. Deficiencies noted included, but were not limited to, failing to use appropriate fall protection systems/devices, not having ladders properly set up and failing to ensure that appropriate personal protective equipment, such as hardhats or safety glasses, were in use at the job site.
