OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flash burn hazards while operating a special processing atmosphere oven without first verifying that adequate engineering controls where in place that would minimize the infiltration of air into the atmosphere, and that would reduce the potential for injury in the event that the oven did backfire: (a) Annealing Area: On or about 11/16/17 employees operating the special processing atmosphere oven designated as "Furnace 3000" were exposed to the hazard of flash burns when the oven was put back into service following repairs without proper verification of the oven's safety controls, including adequate reduction of air from entering the oven's atmosphere and causing backfires that could harm the employees.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Annealing Area: On or about 11/16/17 employees operating and maintaining the special processing atmosphere oven designated as "Furnace 3000" were not provided with appropriate personal protective equipment to protect them from the hazard of flash burns to the face and upper body from the oven backfiring while appropriate engineering controls were being verified.
