29 CFR 1910.23(c)(4): (4) Ladders are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement. On or about November 29, 2017, an employee was exposed to a fall of approximately 15 feet. The employee was using an extension ladder to clear a fallen tree branch from a power line that fed the company lodge on the back of the property. The ladder was not set up on a firm level spot causing it to shift and the employee to fall.
