29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) LSC Communications US, LLC, located at 1145 Conwell Avenue, Willard, OH 44890: On or about November 30, 2017, the employer did not utilize specific procedures to control hazardous energy of the equipment such as, but not limited to, the Goss Sunday 3000 printing press # 392 located in the North Press Room. Employees did not control all hazardous energy sources such as, but not limited to, electrical before performing activities such as, but not limited to, re-webbing/webbing-up the press. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
