On or about January 3, 2018, the employer did not provide a means of safe access and egress to and from the saw operator station on the 7 inch line. The employees have to cross in front of the saw in between runs to reach the operator station.    b.) On or about January 3, 2018, the employer did not provide a means of safe access and egress to and from the saw operator station on the 9 inch line. The employees have to cross in front of the saw in between runs to reach the operator station.     c.) On or about January 3, 2018, the employer did not ensure each employee used a safe means of exiting from the saw operator station on the 9 inch line. The employees were observed climbing through the racks to exit the operator station area.

The employer did not cover the 14 inch wide, 13.5 inch deep hole exposing employees to a trip, slip, or fall hazard.     b) On or about January 3, 2018, the employer did not inspect the walking working surface on the 9 inch line after installing the new press. The employer did not cover the 3.5 inch wide, 13.5 inch deep hole in front of the saw operation station exposing employees to a trip, slip, or fall hazard.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    The employer did not develop and implement a monitoring program for employees exposed to 8-hour time-weighted average (TWA) sound levels of 85 dBA or greater (50%).    a) On January 11, 2018, the 7 inch saw operator was exposed to continuous noise levels at 73.6% of the permissible daily exposure (8-hour, time-weighted average sound level of 85 dBA or equivalently, a dose of 50 percent), during the 349 minute sampling period. Calculations include a zero exposure for the 131 minutes not sampled. The employer had not developed or implemented a monitoring program to assess employee noise level exposures in the area.    b) On January 11, 2018, the 9 inch saw operator was exposed to continuous noise levels at 110.6% of the permissible daily exposure (8-hour, time-weighted average sound level of 85 dBA or equivalently, a dose of 50 percent), during the 472 minute sampling period. Calculations include a zero exposure for the 8 minutes not sampled. The employer had not developed or implemented a monitoring program to assess employee noise level exposures in the area.    c) On January 11, 2018, the 9 inch press operator was exposed to continuous noise levels at 85.0% of the permissible daily exposure (8-hour, time-weighted average sound level of 85 dBA or equivalently, a dose of 50 percent), during the 440 minute sampling period. Calculations include a zero exposure for the 40 minutes not sampled. The employer had not developed or implemented a monitoring program to assess employee noise level exposures in the area.
