29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    (a) B-1 Line dough divider - The platform used to access the hopper, located on the backside of the dough divider, was not equipped with a guardrail or other fall protection system on the ladder access side of the platform exposing workers to a fall hazard of approximately 59 inches to the concrete floor below. On or about January 8, 2018, an employee accessed the platform in order to clear dough from a sensor located on the dough divider.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

(a) B-1 Line, Dough Divider - The standard guardrail system was not capable of withstanding a force of at least 200 pounds in a outward direction. On or about January 8, 2018, an employee ascended a platform to clear dough from a sensor. While descending from the platform, the guardrail detached from the platform allowing the employee to fall to the floor resulting in a serious head injury.  No abatement certification or documentation is required for this item.
