29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    On or about 1/25/2018, at the CV2 conveyor area, employees were exposed to electrical shock and burn hazards while using equipment receiving power from extension cords used in lieu of fixed wiring.  Instances include, but are not limited to:    a. CV2 water cooler receiving power from an extension cord connected to another extension cord (daisy chained)    b. CV2 heat sealer TT4 receiving power from an extension cord.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    On or about 1/25/2018, at the CV2 conveyor area, employees plugging and unplugging equipment and tools were exposed to electrical shock hazards. The knockout box metal-type receptacle used to provide power to a Pacemaker heat sealer (S/N 5617) and to an extension cord used to provide power to the CV2 water cooler, TT3 Pacemaker heat sealer (S/N 5558), and Pacemaker heat sealer (S/N 5561) had a missing knockout.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    On or about 01/25/2018 and at times prior to, in the CV2 area, employees performing service and maintenance on equipment including but not limited to the conveyor lines were exposed to amputation hazards where specific procedures did not include specific steps to shut down the conveyor.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. 29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.     On or about 01/25/2019 and at times prior to, employees performed servicing and maintenance on various machinery and equipment including but not limited to line-shaft conveyors and Peerless E-57 plate machines.  The energy control procedures had deficiencies including, but not limited to the following:    a.  in the CV2 area, employees performing service and maintenance on equipment including but not limited to the conveyor lines were exposed to amputation hazards where specific procedures did not include the specific steps for the removal and transfer of lockout devices and the responsibility for them.     b.  at the Peerless E-57 plate machines, employees performing service and maintenance were exposed to amputation hazards where the specific procedures  did not include specific steps for the removal or transfer of the lockout or tag out devices and the responsibility for them.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Aspen Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(i), which was contained in OSHA inspection number 1206593, citation number 1, item number 4 and was affirmed as a final order on 4/25/2017, with respect to a workplace located at 4231 Clary Boulevard Kansas City, MO 64130.
