29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 22, 2018: a) At the facility located at 3220 Avondale Mill Road in Macon, Georgia employee performing servicing and maintenance activities including, but not limited to making adjustments to the plate machines were exposed to an amputation hazard, when the employer failed to utilize specific procedures for employees performing adjustments to Peerless E- 57 plate machines within the facility.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft: On January 25, 2018; a) Employees working as part of a crew, department or craft while performing service and maintenance on a variety of machinery and equipment including, but not limited to Peerless plate machines were exposed to amputation hazards when the employer failed to ensure all members involved in activities had a personal lock out and tag out device.
