29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer is failing to protect employees from an amputation hazard.  This was most recently documented on January 25, 2018 at 2073 N. Rademakers Way, Fremont, Nebraska.  The employer failed to ensure that the guard on the following provided employee(s)protection from contact with an in-running nip point.  a. In the Pipe department the Duplexing Panel Machine Serial number NOR-24RF-037-18, Model 24RF that was attached to and used in conjunction with the Radial Roller Serial Number NOR-OV10-026 Model 370.  This Radial Roller was equipped with a guard that did not enclose the swing arm on the lower side.  During operations the swing arm would rotate down below the guard and swing in a counterclockwise direct, creating an in-running nip point at the edge of the guard as the swing arm rotated upwards.  b. In the Pipe department the Duplexing Panel Machine Serial number NOR-24RF-042-18, Model 24RF that was equipped with a Radial Roller Serial Number NOR-OV10-029 Model 370. This Radial Roller was equipped with a guard that did not enclose the swing arm on the lower side.  During operations the swing arm would rotate down below the guard and swing in a counterclockwise direct, creating an in-running nip point at the edge of the guard as the swing arm rotated upwards.
