29 CFR 1926.501(b)(13):   Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems:  At a residential construction  worksite 3507 105th Terr, Kansas City, Missouri- Employees were performing roofing operations at an elevated height of approximately 17 feet, without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):   Where portable ladder(s) were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   At a residential construction worksite 3507 105th Terr, Kansas City, Missouri- Employees were accessing a working surface by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
