29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   At the residential worksite located at Lot 1465 NW Mackenzie Drive, Lee's Summit, Missouri:  Employees engaged in roofing activities were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals.  The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work.  Hazardous chemicals used at the site included but were not limited to gasoline.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At the residential worksite located at Lot 1465 NW Mackenzie Drive, Lee's Summit, Missouri:  Employees engaged in roofing activities while using pneumatic nail guns were not protected from flying particles by using eye or face protection.     The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1349437, citation number  1, item number 1 and was affirmed as a final order on 2/15/19, with respect to a workplace located at 6517 Noble Street, Shawnee Mission, Kansas.

The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1290321, citation number 1, item number 1 and was affirmed as a final order on 4/18/18, with respect to a workplace located at 3507 E 105th Terrace, Kansas City, Missouri.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1349350, citation number 2, item number 1 and was affirmed as a final order on 2/15/19, with respect to a workplace located at 6517 Noble Street, Shawnee Mission, Kansas.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the residential worksite located at Lot 1465 NW Mackenzie Drive, Lee's Summit, Missouri:  Employees were accessing a working surface on a pitched roof by way of a portable ladder whose side rails did not extend 36 inches above the upper landing surface being accessed.   The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1290321, citation number 1, item number 2 and was affirmed as a final order on 4/18/18, with respect to a workplace located at 3507 E 105th Terrace, Kansas City, Missouri.
