29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about January 30, 2018, employees performing roofing operations were exposed to an energized, un-insulated, overhead service drop, exposing employees to an electrical shock.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width:     On or about January 30, 2018, employees performing roofing operations at a commercial building were not protected from a fall, exposing employees to a fall of 11.9 feet.     Smart Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA violation number 900637, citation 2, item 1, issued on May 20, 2013, and became final order on or about June 4, 2013, with respect to a workplace located at 835 Pearson St., Des Plaines, IL 60016    Smart Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA violation number 312320492, citation 1, item 4a, issued on August 1, 2008, and became final order on or about August 21, 2008, with respect to a workplace located at 310 Green Bay Road, Winnetka, IL 60093.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
