Fabrication Shop - On or about and times prior to January 30, 2018, the exterior stairway used to gain access into the shop was not equipped with a stair rail system on both open sides of the stairway, thus exposing employees to a trip and fall hazard.

Yard Area - On or about and times prior to January 30, 2018, the employer did not ensure an employee that operated a Komatsu 40 forklift had received formal instruction and evaluated for proficiency, thus exposing the employee to a programmatic hazard.
