29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about January 25, 2018, the employer did not ensure a sanitation worker performing service and maintenance (cleaning) operations on the mozzarella cheese curd line dicer machine implemented energy control procedures by; preparing the machine for shutdown [per 1910.147(d)(1) requirements], turning off the machine [(d)(2)], isolating the machine from the energy source [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to cleaning the machine [(d)(6)].
