29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Cooper Farms, Inc. on or about January 29, 2018, at the address of 1 Cooper Farms Rd. St. Henry, OH: The employer did not ensure the incoming nip point between the rotating helix-shafts (point of operation) contained on the Line #1 Breast Cleaner was adequately guarded. Operating employees were exposed to amputations hazards at the point of operation while manually feeding product into the machine. When performing this task, the operators hand(s) are within the proximity of the incoming nip-point between the rotating shafts. Cooper Farms, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1156600, citation number 1, item number 1 and was affirmed as a final order on August 18, 2016, with respect to a workplace located at 1 Cooper Farms Dr. St. Henry, OH. Cooper Farms, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1017041, citation number 1, item number 2 and was affirmed as a final order on July 16, 2015, with respect to a workplace located at 1 Cooper Farms Dr. St. Henry, OH.
