Employees were exposed to fall hazards greater than 6 feet.

29 CFR 1915.74(a)(4): The employer permitted employees to board or leave a vessel by using a gangway that was not kept properly trimmed at all times:    On February 6, 2018, and times prior there to, USS Harkness, Yard 1, the slip side wall, a gangway was used by employees in conjunction as a stairway to access the vessel.  The gangway contained damaged, bent, inward guardrails in need of trimming, at the end of the gangway, to provide clear access.

On February 6, 2018, at the Slip, Yard 1, Employees used a wire rope for pulling a vessel without the identification markings to indicate a safe work load, exposing employees to struck-by hazards.
