29 CFR  1910.22(a)(3):(3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  On or about October 20, 2021, and times prior thereto, on Ex-Mendocino vessel employees were exposed to slip/trip hazard while gaining access to vessel with the damaged ramp.

29 CFR  1915.82(b)(4):Lights and lighting stringers are not suspended solely by their electric cords unless they are designed by the manufacturer to be suspended in this way;  On or about October 20, 2021, and times prior thereto, on the Ex-Mendocino ship, employees using temporary lighting taped to a handrail to gain access upper/lower deck were exposed to electrical hazards.
