On or about February 7, 2018 through March 7, 2018, the employer had not documented machine-specific lockout/tagout procedures for the control of hazardous energy sources when maintenance employees were assigned servicing and maintenance tasks on the press brakes.   Employees were exposed to crushing hazards likely to result in amputation to hands and fingers from unexpected start up and/or energy release.        Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employee(s) were not trained in and familiar with safety-related work practices such as the use of special precautionary techniques, personal protective equipment (PPE), insulated tools and insulating and shielding materials necessary to safely accomplish such tasks.  Employees were exposed to the hazards of electrocution and arc flash/blast injuries.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
