29 CFR  1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  On or about November 28, 2022, the employer did not perform periodic inspections on the energy control procedures on machines or any other equipment that requires lock out tag out. The periodic inspection shall include a review with each authorized employee of that authorized employee's responsibilities under the energy control procedure.  Employees were exposed to amputation and crushing hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   On or about November 28, 2022, the employer did not ensure that all employees were trained to isolate hazardous energy sources prior to engaging in servicing and maintenance activities. The employee(s) did not have the knowledge and skills to first shut down and lock out electrical energy sources, exposing employees to crushing injuries.  Employees were exposed to moving machinery in the cell of the 10K laser automation equipment.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence:   (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished:  On or about November 28, 2022 employee(s) performing servicing and maintenance tasks were exposed to moving machinery in the cell of the 10K laser automation equipment. Employees did not lock out the machine/cell exposing employees to crushing hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
