29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: In the paint mixing room employees were exposed to toxic or hazardous substances. A powered air purifying respirator (PAPR) was being stored improperly. This was demonstrated when the respirator was left hanging from its straps in the open room with the cartridge still attached. The respirator was exposed to contaminants and damaging chemicals when employees were not using it.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap: In the final assembly area employees were exposed to electrical shock hazards. A spliced flexible cord was in operation at 110 V and was connected to the power supply wired for a manufactured sign in order to test and program the signs.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Employees were exposed to electrical shock hazards associated with cord and plug equipment. The equipment, a Grizzly Metal-Cutting Band Saw (G8144) in the pre-assembly area was in use and was missing its ground pin. b) Employees were exposed to electrical shock hazards associated with cord and plug equipment. The equipment, a flexible extension cord was powered and available for use and showed visible damage to the outer jacketing.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. Employees were exposed to toxic or hazardous substances. In the paint mixing area two containers were in use and were missing a label that indicated their contents. The containers were identified to contain lacquer thinner (OMNI MS251 Multi-Purpose Solvent) used to clean the paint spray guns.
