29 CFR 1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support: (a) On or about March 13, 2025, an employee was not protected from falling in that an extension ladder placed against the narrow edge of the canopy over the diesel pumps was positioned such that deflection under a load did cause the ladder to slip off its support.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: a) This violation occurred on or about March 13, 2025, when an employee was required to use an extension ladder to access a work area and was not protected from falling more than 6 feet to the driveway below in that he was not trained on placement of the ladder.
