Employees were not utilizing any form of fall protection and were exposed to a fall of approximately 9 feet.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:(a) a)Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about January 25, 2018 the employer did not develop, document, and utilize energy control procedures for authorized employees who perform maintenance on the Haskel Air Driven Liquid Pumps (model No. ALC-15). Employees were exposed to amputation injuries in that energy control procedures were not developed prior to removing and replacing the Haskel Pumps. b) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about February 2, 2018 the employer did not develop, document, and utilize energy control procedures for authorized employees who performed troubleshooting tasks on the Haskel Air Driven Liquid Pumps (model No. ALC-15, serial No. 57061109). Employees were exposed to amputation injuries in that energy control procedures were not developed prior to troubleshooting the Haskel Pumps.

Employees were exposed to amputation injuries in that pneumatic energy sources were not isolated. b) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about February 2, 2018 the employer did not ensure that energy control application steps to control pneumatic energy sources were implemented prior to troubleshooting tasks on the Haskel Air Driven Liquid Pumps (model No. ALC-15, serial No. 57061109).
