CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  a)	Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about May 4, 2021 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to engaging in maintenance and paint cleaning in the e-brite line paint line 1 basecoat booth. Employees entering the paint line booth were exposed to hazardous solvents including Acetone, Butyl Acetate, Isopropyl Alcohol, Diethylene glycol monobutyl ether, Ethanol, 2-butoxy , Glycine, N,N'-1,2-ethanediylbis[N-(carboxymethyl)-, sodium salt (1:4), -Hexadecanamine, N,N-dimethyl-, N-oxide, 2-propanol, Sodium hydroxide (Na(OH)), and propane. Those chemicals can pose a wide range of health and physical hazards such as severe irritation, sensitization, and harm to the eyes (stinging, tearing, redness, swelling, and blurred vision.), skin, specific target organ toxicity, single exposure category 3 narcotic effects, intoxication, drowsiness, dizziness, headache, nausea, and vomiting. Prolonged inhalation may be harmful, may cause irritations to the respiratory tract.

29 CFR  1910.134(d)(1)(i):The employer did not select and provide an appropriate respirator based on the respiratory hazard(s) to which the worker is exposed and workplace and user factors that affect respirator performance and reliability.  a)	Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about May 4, 2021 employees were exposed to  hazardous solvents containing Acetone, Butyl Acetate, Isopropyl Alcohol, Diethylene glycol monobutyl ether, Ethanol, 2-butoxy , Glycine, N,N'-1,2-ethanediylbis[N-(carboxymethyl)-, sodium salt (1:4), -Hexadecanamine, N,N-dimethyl-, N-oxide, 2-propanol, Sodium hydroxide (Na(OH)), propane and its vapor without appropriate respiratory protection while engaging in maintenance and paint cleaning in the e-brite paint line 1 basecoat booth. Employees performing the regular weekly cleaning inside the paint booth were not assigned any type of respirator with filtering face piece that provides protection against chemical vapors. Those chemicals can pose a wide range of health and physical hazards such as severe irritation, sensitization, and harm including intoxication, drowsiness, dizziness, headache, nausea, and vomiting. Prolonged inhalation may be harmful, and may cause irritations to the respiratory tract.

The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures.  a)	Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about May 4, 2021, the employer failed to ensure that each affected employee use protective footwear when working in the e-brite line area where there is a danger of foot injuries due to falling or rolling parts racks.

CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a)	Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about May 4, 2021, the employer did not ensure that energy control application steps to control pneumatic energy sources were implemented prior to performing maintenance and paint cleaning in the e-brite line paint line 1 basecoat booth. Employees entering the paint line booth were inside the working zone of the robotic arm of an energized Yaskawa spraying robot, thereby exposed to struck-by hazards and rotating equipment injuries in that energy sources were not isolated.
