29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    Breitinger Company - Mansfield, Ohio:  On or about February 10, 2018, the employer did not ensure that employees who setup and maintain Body Line machinery were trained in the skills required for the safe application, usage, and removal of energy controls.  Employees who serviced the transfer table, mandrel, and die head were not trained to isolate and/or render energy sources safe.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Breitinger Company - Mansfield, Ohio:  On or about February 10, 2018, employees were exposed to pinch point hazards in that the Body Line sheeter transfer machine was not guarded to protect employees as they loaded the machine and removed skids from the transfer table.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Breitinger Company - Mansfield, Ohio: On or about December 13, 2017, employees were exposed to crushing and pinch point hazards in that the  Body Line sheeter transfer machine was not deenergized prior to servicing tasks such as, but not limited to, maintaining the plunger mechanisms on the transfer arm.  The employer did not ensure that energy control application steps were implemented to control hazardous pneumatic and mechanical energy prior to the servicing tasks - the employer did not:  - Ensure employees had the method or means to control hazardous energy.  - Ensure that equipment was turned off or shut down.  - Physically operate energy isolating devices to control the energy source(s).   - Verify that isolation and de-energization of the machine or equipment had been accomplished.    b. Breitinger Company - Mansfield, Ohio: On or about February 10, 2018, employees were exposed to rotating part and pinch point hazards in that the  Body Line was not deenergized prior to conducting a changeover of the mandrel and die head.  The employer did not ensure that energy control application steps were implemented to control hazardous pneumatic and mechanical energy prior to the setup tasks - the employer did not:  - Ensure employees had the method or means to control hazardous energy.  - Ensure that equipment was turned off or shut down.  - Physically operate energy isolating devices to control the energy source(s).   - Verify that isolation and de-energization of the machine or equipment had been accomplished.    Breitinger Company was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1072998, citation number 1, item number 3 and was affirmed as a final order on September 3, 2015, with respect to a workplace located at 595 Oakenwaldt St., Mansfield, OH 44905.
