29 CFR 1910.132(d)(1)(iii):The employer did not select PPE that properly fit each affected employee: a) Breitinger Company located at 595 Oakenwald Street, Mansfield, Ohio, 44905: On or about September 14, 2023, the employer failed to select personal protection equipment (PPE) that properly fit each employee tasked with operating the Hurricane Parts Washer located in Building D. The employer provided loose fitting cotton gloves that created caught in hazards at in going nip point created between the conveyor and the chute at the unloading end of the Hurricane Parts Washer, resulting in amputation injury.

29 CFR 1910.132(f)(1):The employer did not provide training to each employee who is required by this section to use personal protective equipment: a) Breitinger Company located at 595 Oakenwald Street, Mansfield, Ohio, 44905: On or about September 14, 2023, the employer failed to provide training on proper selection, maintenance and usage of personal protective equipment the employer required for all Hurricane Parts Washer operators. An employee was provided personal protective equipment (gloves) that did not fit properly which exposed the employee to caught in hazards while operating the Parts Washer in Building D. Employee's gloves became caught in the in going nip points created between the running conveyor and chute at the unloading end of the equipment which resulted in amputation injury.

The employer failed to ensure that the energy control procedures were utilized to control the electrical energy sources prior to the employee performing this task.
