29 CFR 1926.95(a): Protective shields and barriers were not provided, used, or maintained where necessary by reason of hazards of processes or environment causing injury or impairment in the function of any part of the body through physical contact: A.) Perry Construction Group, Inc., personal care storage/dock addition, on or about February 13, 2018 and times prior thereto: Employees were exposed to impalement hazards when walking/working near structural steel connecting bolt that were protruding vertically out of concrete pillars. The bolts were not shielded by protection caps. The bolts were 32-44 inches from ground level.
