29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, solid panels or equivalent intermediate members were not installed between the walking/working surface and the top edge of the guardrail system: A.) GeorgeKo Industries, Inc.; outside office - On or about 2/8/18, and at times prior thereto, a stairway that led to the mezzanine area did not have a mid rail. The stairway was approximately 10 ft. from the mezzanine to the floor.
