29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:(a)  a.)  Georgeko Industries Inc. - On or about 1/13/25, and times prior thereto, the forks on the forklifts had holes drilled into them to allow a hoist to be utilized without the forklift manufacturer's approval.  b.)  Georgeko Industries, Inc. - On or about 1/13/25, and at times prior thereto,  a man basket was being used on a Toyota forklift  (serial # 20787) without approval from the forklift manufacturer's approval.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:     a.)  Georgeko Industries Inc. - On or about 1/13/25, and times prior thereto,  a hoist attachment was being used on various forklifts without the forklift plate being changed to identify the attachment and load changing information.  b.)  Georgeko Industries, Inc. - On or about 1/13/25, and at times prior thereto,  a man basket was being used on a Toyota forklift  (serial # 20787) without the forklift plate being changed to identify the attachment and load changing information.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive initial training on fork and attachment adaptation, operation, and use limitations; prior to operating the forklifts:   a.)  Georgeko Industries Inc. - On or about 1/13/25, and times prior thereto,  various attachment were being utilized with various forklifts.  Attachments included, but were not limited to, a man basket and a hoist.  The forklift operators had not been trained on the attachments prior to operating the forklifts with the attachments.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a.)  Georgeko Industries, Inc. - On or about 1/13/25, and at times prior thereto, employees are standing on top of the presses to clean the presses and loaders and to fill the presses with plastic pellets without fall protection.  Employees are 5-6 feet off the ground when standing on the presses.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
